Explore published concerns by place. See what is recorded, what the evidence supports and what remains unknown. A report is not a verified finding.
Place knowledge does not mean reporting is open or that every concern is represented.
This place filter is not valid. Clear the filters and choose a state, then a local area.
Most recently updated first. Search also works with a public issue reference or institution name.

Only issues approved for safe public sharing will appear here. An empty registry does not mean your community has no problems.

Why a response is not yet a resolution
These labels describe recorded evidence. They are not a guaranteed sequence of progress.
- Reported
- A concern has been recorded. It is not yet a verified finding. Several observations may relate to one issue; their number is not proof.
- Under review
- The concern is being checked. Responsibility guidance does not mean an institution has been contacted.
- Waiting for a response
- Read the action record to see what was actually sent. Prepared is not sent; sent is not acknowledged. An acknowledgement alone is not a substantive response.
- Response received
- An institution response has been recorded. A response is not proof that the problem has been fixed.
- Partly addressed
- The recorded outcome covers part of the issue. Some concerns remain.
- Resolved
- The recorded evidence supports resolution. A reply or a high report count alone cannot establish this.
- Reopened
- Follow-up has resumed. Earlier events remain part of the history.
- Follow-up due
- A follow-up step is due. This does not establish that an institution has responded or that the issue is resolved.
- Follow-up closed; resolution not verified
- Follow-up has ended without verified resolution. Closed does not mean resolved.
